Agora is guided by the Health Services Act 1988 (Vic), the Privacy Act 1988 (Cth) (and its Australian Privacy Principles) and the Health Records Act 2001 (Vic) (and its Health Privacy Principles) and apply them to our relationship with patients, employees, students, and service providers.
Agora collects information from patients in order to provide health care services to them. If you are a patient treated by us or intend to be, you will have collected your personal information regarding: name, date of birth, address, contact details, Medicare and private health insurer, financial details, ethnic background, health and medical history, lifestyle history, family history, details regarding your current health issue and details regarding your treating doctors.
Agora also collects personal information from its employees, students, job applicants, traders, service providers and contractors. The information collected is required so to enable us to assess, work and contract them.
In case you do not wish Agora to collect your personal information we may not be able to service and or contract with you. We collect your personal information directly by you or occasionally under your permission and/or under the limits of the law, from a third party such as from any other treating doctor looking after your health, from your family and or carer. Any information received but not required by Agora is destroyed or de-identified.
